This section explains how we may use and disclose your PHI and your rights under the Health Insurance Portability and Accountability Act of 1996 ("HIPAA").

  4. Request Restrictions on certain disclosures (we are not required to agree, except for self‑paid services where you request nondisclosure to insurers).
  5. Request Confidential Communications (e.g., alternate address).
  6. Receive a Paper Copy of this Notice upon request.

Submit a written request to the Privacy Office at the contact details above. We will respond within 30 days (60 days for off‑site records), or as state law requires.

If you believe your privacy rights have been violated, you may file a complaint with our Privacy Office or with the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate for filing a complaint.

We may change this Notice at any time. When we do, the new Notice will apply to all PHI we maintain and will be posted on our website.
