How Our 2–4 Day Protocol Works

Your accelerated TMS journey is broken into the following steps. Every patient begins with a thorough evaluation, including a psychiatric intake and qEEG brain map to identify the optimal stimulation site. Then, over 2–4 days, you’ll receive 10 brief but potent sessions of TMS per side each day—totaling 40 sessions. Each stimulation burst lasts about three minutes; between bursts, you’ll participate in short VR exercises to help your brain integrate new patterns. On the final day, we repeat the qEEG to measure progress and discuss next steps.

What is accelerated TMS for ADHD?

Accelerated Transcranial Magnetic Stimulation (TMS) for ADHD is a non-invasive neuromodulation treatment that delivers 20–40 short magnetic pulses per day to brain regions responsible for attention and impulse control. Instead of one session per day over many weeks, our accelerated protocol condenses the entire course into just 2–4 consecutive days. Each day, you receive 20 sessions on the left side and 20 sessions on the right side (40 total), or fewer depending on your personalized plan. This intensive, cash-pay protocol is designed to jump-start neuroplasticity and lead to faster improvements in focus, organization, and self-regulation than traditional schedules.

How does the 2–4 day accelerated protocol work?

Day 0 (Intake & Mapping): You meet with Dr. Rubaye for a comprehensive psychiatric evaluation and qEEG brain mapping to identify the optimal stimulation target.

Day 1–3 (Treatment Days): Each morning, you arrive for your TMS sessions. You’ll receive 20 stimulation bursts on one side of the prefrontal cortex (3 minutes each), followed by a VR cognitive training session during the break. Then 20 bursts on the opposite side, again paired with VR exercises during rest periods. Each day totals 40 stimulation runs plus VR work, completed in about 4–5 hours.

Day 4 (Final Day & Follow-Up): You complete your last set of TMS + VR sessions, followed by a brief qEEG to measure immediate changes. Dr. Rubaye reviews your progress and outlines any needed follow-up.

Days 7 : We conduct brief telehealth check-ins, including optional repeat qEEG at one month, to ensure the improvements are lasting and to discuss any booster needs.

Who is a good candidate for accelerated TMS for ADHD?

You may qualify if you are:

An adult (ages 18–70) diagnosed with ADHD, particularly if you have not benefited sufficiently from stimulant medications or behavioral therapy.

In generally good health and willing to participate in an intensive 2–4 day protocol.Free of contraindications such as implanted metal or electronic devices in the head (e.g., cochlear implants, aneurysm clips, deep brain stimulators) and without a history of uncontrolled seizures or epilepsy.Willing to commit to a cash-pay program, as we do not bill insurance for accelerated TMS.

Most psychiatric medications—including ADHD stimulants—can be continued during treatment. If you take high-dose benzodiazepines, Dr. Rubaye may recommend a brief taper to maximize TMS effectiveness.

How quickly will I notice improvements in my ADHD symptoms?

Because accelerated TMS delivers 40 stimulation sessions over 2–4 days, many patients report noticing subtle improvements in focus, mental clarity, and impulse control within the first two days of treatment. By the end of Day 3 or Day 4, most people experience a significant reduction in distractibility and an ability to sustain attention more effectively. Continued gains often appear over the subsequent week as your brain’s networks continue to reorganize.

Is accelerated TMS for ADHD safe? What are the side effects?

Yes. Accelerated TMS is non-invasive and does not require anesthesia. Common, mild side effects include:

Scalp discomfort or mild headache: Usually occurring only on Day 1 or Day 2 and easily managed with over-the-counter pain relievers.

Temporary fatigue: More mental than physical; many patients describe being “mentally tired” after a full day of TMS + VR.

Scalp twitching: A normal reaction as the magnetic pulses activate nerve endings, but not painful.

Serious adverse events such as seizures are extremely rare (under 0.1 %) when proper screening is done. There are no systemic side effects—no sedation, no cognitive fog, and no long-term memory changes.

How long do the benefits last, and will I need booster sessions?

Most patients maintain improved attention and executive control for at least six months following the accelerated TMS + VR protocol. In our follow-up data, about 70 % of patients still hold at least half of their gains at six months. If ADHD symptoms return or new challenges in focus emerge, you can opt for a one-day booster consisting of 10 additional TMS sessions (per side) or a two-day booster with 20 sessions. Boosters typically restore full benefits quickly and are offered at a reduced cost compared to the initial protocol. Many patients need only one booster per year, if any.
